Lost Card Replacement
CPR Card - First Aid Card
If you have lost your CPR or First Aid card (within one year of issue), we can
look in our records and create you a new card. However, this card replacement
request can ONLY BE DONE BY MAIL, may take a couple of weeks, and the cost is:

To request your replacement card, you must enclose ALL THREE of these items in an envelope:
NOTE WITH YOUR NAME, DATE OF COURSE, AND TYPE OF COURSE
REPLACEMENT FEE (CASH OR CHECK)
SELF ADDRESSED STAMPED ENVELOPE
